My Date of Birth Is Incorrect on the Prescription

Your personal information, including your date of birth (DOB), is entered into SignatureRx by your GP based on the records they hold for you.

If your DOB is incorrect, you may experience issues collecting your medication at a pharmacy. Pharmacies usually need both the prescription ID and your DOB to view and dispense your prescription.

If this happens, you will need to contact your doctor or clinic. They can void the incorrect prescription and re-issue it with the correct information so you can collect your medication without problems.
